Simon Cowell has been candid about the work he’s had done.
The music mogul, who first rose to fame in the early 2000s as a judge on American Idol, has undergone a handful of treatments and cosmetic procedures over the years — including a few he’s regretted. During an April 2022 interview with The Sun, Cowell admitted he had gone overboard with cosmetic work at one point, so much so that he didn’t recognize himself.
“There was a stage where I might have gone a bit too far. I saw a picture of me from ‘before’ the other day, and didn’t recognize it as me first of all,” he said, adding that many people in Hollywood were “having their faces pumped full of this and the other” at the time.
These days, the X Factor alum sticks to a healthy diet and exercise to maintain his appearance, as well as a controversial wellness treatment.

He’s gotten Botox and filler

While Cowell hasn’t spoken publicly about any major cosmetic procedures, the America’s Got Talent star has been open about his use of Botox injections and filler.
“I’ve had Botox. I’ve had lasers and this and that,” he told the Daily Mail in December 2025. “I look at myself sometimes and go, ‘Oh God, I look s----.’ Sometimes I think I look okay. I’m more about, ‘Is it real?’ I guess that authenticity was my main thing.”
Cowell denied undergoing a facelift in the same Daily Mail interview.
He gets blood cleansing treatments

Cowell has also tried a controversial blood-filtering treatment. He opened up about the procedure during a December 2025 interview with British GQ, likening the experience to “going into Frankenstein's laboratory.”
“I went to a wellness clinic in Spain called The Sha ... they have a machine, which filters, I think it's either your blood or blood plasma,” he told the outlet. “It comes out of your body through these tubes, takes about three and a half hours, and it apparently removes all the toxins.”
He added that the procedure was “really painful,” but was something he knew he wanted to do as soon as he heard about it.
“Whether you are inhaling, I don't know, bad pollution or whatever, I'm sure that pretty bad things are in our blood system,” he continued. “So I'm now a great believer of — age from the inside rather than the surface. Botox is great, but it's what's on the inside that really matters.”
Cowell did not reveal the name of the procedure, but similar blood-filtering treatments have been gaining popularity among celebrities. In July 2025, Orlando Bloom revealed that he’d undergone a blood “cleaning” treatment to rid his body of microplastics.
However, there haven't been any meaningful studies studies that prove these treatments’ efficacy.
He briefly considered cryonics

Another cutting-edge procedure Cowell considered was cryonics, which is the practice of preserving bodies through ultra-low temperatures in the hope that future technology can revive them.
“I did think for a while: We’re all going to eventually, unfortunately, die. What if then I’ll just freeze myself?” he told The New York Times in November 2025.
Cowell’s perspective changed, however, after he became a dad. He continued, “Then someone told me they chop your head off, and I think your brain’s frozen. So then the idea of coming back as some kind of robot in 3,000 years time, it’s like: Forget it.”
He stopped getting injections for his son

Cowell’s son, Eric, is also the reason he stopped getting facial filler. During his interview with The Sun, the television personality reflected on the moment he realized he had gone “too far” with filler.
“Eric was in hysterics,” he said, noting that he looked like “something out of a horror film.”
“Enough was enough. There is no filler in my face at all now. Zero,” he said.
Now, Cowell focuses on health eating and increasing his water intake, telling The Sun in November 2025 that he's "aged backwards" thanks to "more exercise, less stress, and certain supplements."
The shift to a healthier lifestyle was also influenced by his 2020 e-bike accident, during which he broke his back.
“It was painful, it wasn’t great,” he said of the accident during a May 2023 appearance on Today. “However, I was so unfit before the accident. I didn’t know how unfit I was until I had to do all this stuff afterwards," he explained. “And I’m like, ‘My God.’ Things like that, I believe happened for a reason.”
